The following checklist may be used to gather information immediately following your accident and may be helpful in the event you need to retain an attorney.

It is important to ask the right questions. It all starts with who, what, when, where and why.
* What happened? What type of accident where you involved in?
* Where did the accident take place? (If you are involved in an auto accident it is important to note the placement of the vehicles.)
* Who was at fault for causing the accident? Be sure to get names, addresses and phone numbers from anyone who was involved in or witnessed the accident.
* Did the police come and was anyone charged (if so with what)?
* Are there any known witnesses (if so, who)?
* Did the responsible party make any statements (if so what was said)?
* Have you made any statements to anyone (if so what did you say)?
* Has anyone claimed you were at fault for causing the accident (if so who claimed what)?
* Were alcohol or drugs involved in any way with any person?
* Were there any tangential factors involved in the cause of the accident (natural factors such as inclement weather, ice, bright sun, animals and insects, or pedestrians, mechanical failure, sudden emergency, act of God)?
* If the responsible are unknown, was a “John Doe” report made to the police or insurance company?
Documentation Beats Conversation:
It is important to save any documentation of your accident and injury. These items may include police reports, photographs, medical records and insurance documents. Be sure to get copies of any statements in writing.
