Need to change the pattern of Medical Education & Syllabus in our Country.

In our country,We adopted the Modern System of medicine through the British pattern.In the early part of preindependence era & subsequently we had a couple of medical Schools imparting the LCPS degree which subsequently changed to M.B.B.S.Since then over the past 60+ years we have continued with the same system of basic learning without giving much thought to the need of changing times, techonolgy & the countries needs.To become a Doctor practicing modern medicine you have to join the college after the age of 18 years. You have to complete the 10+2 pattern of schooling & then join a course stretching over four & half years of Academic learning & one years internship.After completing the grind you an " M.B.B.S." qualified Doctor supposed to start practicing the profession. The education as is imparted today has almost lost complete releavance to the needs of Modern times.A young man is qualified by degree as a Doctor but he hardly posseses the adequate practical knowledge to apply this knowledge in practice. A young Modern medicine graduate with an M.B.B.S degree hardly has any confidence to start in to practce of medicine.Just check the scenario with other professional degrees , all other vocational degrees turn out a specialist at the graduate level , e.g. B.E. in Mettalurgy, BSc.in Biotechology & look at our degree A General degree with hardly any speciality training! nothing wrong with that since we are supposed to deal with human life & not material inanimate objects, but the tragedy of the situation is that this Generalist hardly has any general , leave apart any special knowledge.The entire Medical syllabus for an M.B.B.S. degree puts more stress on Academic leaning than practical learning.The Internship after graduation examination makes you forget what you have learnt in books rather than teach you anything useful practically.What is required is as follows , as i can find out from the revised syllabus for the 10 th standard exam. a boy or a girl are sufficiently enligtened about the Human Anatomy & Physiology. Why are we insisting on these poor creatures to waste two more years in a Junior college ? Why can not a sudent join his medical college after 10th Exam. ? why on earth we have to complete the age of 18 years....it is understandable that by the time you graduate to be Doctor you should be legally Major ( Read 18 yrs of age),but why not join prior to that age? By doing this type of arrangement we will be achieveing three or four important objectives,,,

1.The student joing after 10 the exam , a medical course will complete the course ( with ofcourse a revised & revamped syllabus!) in the same four & half years as is today , but after the Final M.B.B.S. Examination , instead of the present stupid system of one years useless "Internship" he will be posted as a Junior Houseman with full responsibility(& Ofcourse Stipend!) & rotated for six months terms in the basic clinical sciences of a) Internal Medicine b) General Surgery c) Gynecology & Obstetrics d)Paediatrics e) Community Medicine f)Casualty & Emergency Medicine.. by undergoing training in this way this chap will have been better equipped to deal with Medical Practice & would have completed the course in the same time span but with good practical knoewledge.

2. The nomenclature of calling a Doctors degree M..B.B.S. is outdated to my mind, let us now do away with this , instead let the basic degree be M.D. in Medicine.

3. The present system of post graduate learning too has lost relevance ,whole lot of Diplomsa , M.D.'s & M.S.'s & to add to that the subsequent superspecialisations of D.M.'s & M.C.h.'s are awarded by various Universities .For this to acquire ,you have to get admission after a stupid superspeciality entrance exam. which is a test of elimination rather than a test of selection! After having acquired my M.S. in General surgery almost 27 years I fail to understand as to why my Shivaji university gave me that degree & why at all I accepted it. Who had given them that authority? In fact if you go to the root of Post graduate learning , it is apparent that it has to be in the format of "Guru-Shishya parampara" a great master teacher creating his duplicate! but for me my teacher was hardly a master he was a teacher only because by seniority & having stayed in a Government job he was promoted as a Professor & not because he had the merit! In the present system of Post graduate teaching only those Doctors who happen to be in the Medical colleges teaching faculty ( & usually they are there by compulsion of their circumstances & not by their choice!) happen to acquire the garb of a P.G. teacher ....here your practical experience & Merit has no relevance.How can these "Teachers" produce master craftsmen? ... things have come to such a ridiculous pass that every year after year these so called Government teacher are either retiring of going out for private lucrative jobs & as such every year the number of seats for intake in speciality & superspeciality are dwindling & we the emunent practicing fraternity keep on lamenting & shouting about the state of affairs & blaming the Government! My dear colleagues we have created this situation of our own making. The Asoociation of chartered accountants in our country gives its own degrees , it appoints its own teaachers from amongst the best C.A.'s practicing in this country..... where are our own associations of Physcians, Surgeons, OB.Gy people , why are they sleeping over this very important issue for so long....why I as a Surgeon was not qualified by the Association of Surgeons of India, Who was Shivaji University to give me this degree? The solution to all these problems is simple let all of us as masters in practicing medicine start off with accepting our role as teachers for tomorrows specialists, let the Government & these Medical Colleges be relieved from such an important task. After having acquired the above mentioned M.D. as the basic degree,let all of our indivisual specialist & Superspecialist associaitions start awarding fellowoships in their respective arts & sciences. Let these associations decide upon the Teachership appointing criteria, the examination criteria & qualifying criteria. Let all the Masters in the Practicing side of medicine have a chance of creating their duplicates! Such a scenario will at once achieve many things, 1.The Real Master will ahve a chance to be P.G. teachers.

2.The role of already scarce "Government Teachers" will be made redundant.

3.The need of creating reservations for various casts &n subcsta will no longer felt beacuse suddenly a whole traesure lot of P.G. techers will be available across the country.

4. The confusing nomemclature of Diplkoms, M.D.'s.,M.S.'s, D.M.'s, M.C.h.'s will be done away with , it will be replaced by Fellowships (e.g. FICGS = Fellow of Indian college of General Surgeons, FICOS = Fellow of Indian college of Orthopedic Surgeons, FICIM = Fellow of Indian college of Internal Medicine etc.)

I am sure I have raised over here a very pertinent & relevant topic which will open up a huge dicussion forum , a brain storming of sorts & will lead us all to a new & better direction. Our aim at the end as a medical fraternity is to provide the best possible care for our patients. NO Compromise!
